If you no longer have the original reference number, you will not be able to submit your information as an update. Please use the Start New Report button and note in the text that your information relates to a previous submission.
If you submitted your report using a temporary anonymous account (by saving your report and returning later to submit it), you must sign in to that same temporary anonymous account to add further information.
If you no longer have access to your temporary anonymous account, you can email your additional information to onlinereporting@nhscfa.gov.uk, quoting your report reference.
